Institutional Dysfunctions of Digital Transformation in Healthcare: A Critical Analysis of the Regulatory Model

The article provides a critical analysis of the regulatory model for the digital transformation of Russian healthcare. Based on an analysis of federal strategic documents, industry legislation, and bylaws, latent dysfunctions in the institutional framework for digitalization are identified. It is shown that the expansion of interdepartmental cooperation within the framework of the Unified State Information System in the Field of Healthcare (EGISZ) without adequate protection of personal data undermines the trust of citizens. The accelerated introduction of telemedicine without taking into account socio-psychological barriers and the preservation of alternative channels leads to rejection of innovations, especially in older age cohorts. The unified regulatory model applied to all regions does not take into account significant differences in the level of budget provision, quality of infrastructure and human resources, which creates risks of deepening inter-regional inequality. The need for a transition from a technocratic to a human-centered regulatory model is substantiated, taking into account regional differentiation and providing for the preservation of hybrid access channels.
