The goal of investigation was to study the clinical characteristics and outcomes of infiltrative breast cancer with basal cell differentiation. Materials and methods We investigated the immunohistochemical characteristics of tumors from 119 patients with infiltrative breast cancer. The group of patients included: infiltrative breast cancer with basal cell differentiation (n=23), triple negative phenotype without basal differentiation (n=37) and luminal A+B phenotype of cells (n=59). Comparative analysis of tumor size, clinical stage, frequency of metastasis and clinical outcomes in these three groups of patients were carried. Results: For the infiltrative breast cancers with basal cell differentiation were typical dimensions of the tumor more than 2 cm and the high incidence of hematogenous metastases, which are detected in 43.5% of patients. There are significantly higher than in patients with infiltrative breast cancer with the luminal cell differentiation and triple negative phenotype. The most common sites of metastases are the liver (26%), bones (26%) and lung (21%). Half of the patients with infiltrative breast cancer with basal cell differentiation had clinical stage of the disease II A-b. For these patients early (stage Тis) and late (IIIC and IV) stages of the disease were not detected. Overall 5 year survival rate 73,7% was observed for patients with infiltrative breast cancer with basal cell differentiation This is on 20% less than value for patients with infiltrative cancer with luminal (A+B) phenotype, and on 6% less than value for patients with infiltrative triple negative cancer phenotype. We determined that main factors for adverse clinical outcome were: hematogenous metastases
