Когнитивные нарушения в отдалённом периоде после тотального облучения в малых дозах (к 40-летию Чернобыльской катастрофы)

Цель исследования: выявление причинно-следственных связей между изменениями церебральной перфузии вследствие облучения в малых дозах, сопутствующими факторами и когнитивной дисфункцией. Материалы и методы. Обследовано 66 человек сопоставимого возраста, из них основная группа (ГО) – 47 ликвидаторов последствий аварии (ЛПА) на ЧАЭС, в том числе 35 ЛПА 1986 г. и 12 ЛПА 1987-88 гг. участия, дозы облучения – от 0,09 до 0,36 Гр. Группа сравнения (ГС) – 19 человек. Состояние церебральной перфузии оценивали методом МРТ контрастная DSC-Т2*-перфузия по параметрам: объёму церебрального кровотока (CBV, cerebral blood volume), скорости церебрального кровотока (CBF, cerebral blood flow) и среднему времени прохождения крови (MTT, mean transit time). Когнитивные функции оценивали с применением нейропсихологических тестов: MMSE, MoCA и HADS, состояние сердечно-сосудистой системы – по общепринятым показателям. Для статистического анализа использовалась программа STATISTICA 10.0. Результаты. Среди ЛПА чаще встречается умеренная деменция (RR=1,92; p=0,002) и умеренные когнитивные нарушения (RR=1,19; p=0,005). Основное влияние на развитие деменции оказывают возраст (β=-0,503; p<0,001) и CBV в коре левой лобной доли (β=-0,381; p=0,026). Когнитивный дефицит связан c CBF в коре левой лобной доли (β=-0,379; p=0,014) и индексом Рорера (β=-0,300; p=0,058). Выраженность тревоги у ЛПА связана с CBV в белом веществе (β=-0,177; p=0,093), CBF в коре правой лобной доли (β=0,173; p=0,096) и возрастом (β=0,207; p=0,060), выраженность депрессии – с CBV в белом веществе правой лобной доли (β=-0,316; p=0,002). В ГС деменция связана с избыточной массой тела (β=-0,372; p=0,035), систолическим (β=0,415; p=0,027) и средним артериальным давлением (β=0,409; p=0,049), когнитивные нарушения – с возрастом (β=-1,083; p=0,001), пульсовым артериальным давлением (β=-0,541; p=0,017), CBV в белом веществе (β=-0,702; p=0,003) и коре (β=0,453; p=0,033), тревога – с систолическим (β=-0,535; p=0,016), а деменция – со средним артериальным давлением (β=-0,516; p=0,029). Заключение. Выявленная у ЛПА на ЧАЭС межполушарная гетерогенность когнитивных нарушений и аффективных расстройств, вероятно, связана с радиационным воздействием. В группе сравнения когнитивная дисфункция обусловлена возрастными изменениями и артериальной гипертензией.

Cognitive impairment long after low-level total body irradiation (40 years since the Chernobyl accident)
The purpose of the study is to identify cause-and-effect relationships between changes in cerebral perfusion after low-level irradiation and associated factors and cognitive impairment. Materials and methods. We examined 66 people of comparable age, including 47 liquidators of the Chernobyl accident (the study group; 35 of them participated in the emergency response in 1986 and 12 were active in 1987–1988; they received radiation doses ranging between 0.09 Gy and 0.36 Gy) and 19 individuals from the comparison group. To assess cerebral perfusion, we used DSC T2* MRI perfusion, measuring the following parameters: cerebral blood volume (CBV), cerebral blood flow (CBF), and mean transit time (MTT). Cognitive functions were analyzed using neuropsychological tests (MMSE, MoCA, and HADS); the cardiovascular system was assessed using conventional parameters. Statistical analysis was performed with the help of STATISTICA 10.0. Results. In the study group, the most common types of cognitive impairment are moderate dementia (RR=1.92; p=0.002) and moderate cognitive impairment (RR=1.19; p=0.005). The primary factors influencing dementia are age (β=-0.503; p<0.001) and CBV in the left frontal cortex (β=-0.381; p=0.026). We found a correlation between cognitive deficiency and CBF in the left frontal cortex (β=-0.379; p=0.014) and Rohrer’s Index (β=-0.300; p=0.058). Also, a correlation was found between the levels of anxiety and CBV in the white matter (β=-0.177; p=0.093), CBF in the right frontal cortex (β=0.173; p=0.096) and age (β=0.207; p=0.060), as well as between the severity of depression and CBV in the white matter of the right frontal lobe (β=-0.316; p=0.002). In the comparison group, there is a correlation: between dementia and overweight (β=-0.372; p=0.035), systolic (β=0.415; p=0.027), mean arterial pressure (β=0.409; p=0.049); between cognitive impairment and age (β=-1.083; p=0.001), pulse pressure (β=-0.541; p=0.017), CBV in the white matter (β=-0.702; p=0.003) and the cortex (β=0.453; p=0.033); between anxiety and systolic blood pressure (β=-0.535; p=0.016); between dementia and mean arterial pressure (β=-0.516; p=0.029). Conclusion. The hemispheric asymmetry in cognitive impairment and affective disorders found in the liquidators of the Chernobyl accident is probably caused by their exposure to radiation. In the comparison group, cognitive deficiency is due to age-related changes and hypertension.
