The concept of clinical trials in hematology integrated into real practice

Clinical research is one of the mainfactors in obtaining scientific information on various problems of modern medicine. They represent models of real clinical practice and are divided intoseveral options with varying degrees of evidence.Clinical research integrated into real practice, basedon the nonselective principle of patient groupformation, is important for practical health care.This article presents the results of own publishedstudies integrated into real practice in comparison with published results of similar randomizedstudies. Treatment of relapse/refractory multiple myeloma with bortezomib as well as combinationof drugs - lenalidomide, bortezomib and prednisolone showed that own programs were less toxicdue to lower cumulative doses of drugs with almostthe same immediate and distant results. The sameresults of comparative studies were obtained whenanalyzed the programs of bortezomib, melphalan,prednisolone at the first detected MM. Chronicmyeloid leukemia treatment in our own studiesshowed that the reduction of cumulative dosesworsened the quality of the response, but did notaffect the overall survival over the same observation intervals. On the basis of the obtained resultsthe concept of clinical trials integrated into realpractice was formulated, in which the conditions ofpractical health care were reproduced. The resultsof these studies significantly expand informationabout new drugs, their combinations and treatment programs obtained in randomized studies.
