We consider the internal picture of disease and particularly its motivational component in patients with chronic renal failure and diabetes as a disease with a vital threat, requiring patients to the formation of adherence to therapy. The features of value-semantic and motivational sphere of patients in the disease situation and how they differ from receiving hemodialysis patients with renal failure, indicating the formation of the manifestations of their adherence to treatment in a situation of vital threat.
