Почечный канальцевый ацидоз как одна из причин мочекаменной болезни

Введение. Изучение причин литогенеза при мочекаменной болезни (МКБ) крайне важно как для определения тактики хирургического лечения, так и для метафилактики уролитиаза. Материалы и методы. При составлении обзора литературы использовались данные, опубликованные в базах PubMed, научной электронной библиотеке eLibrary.ru, на сайте Европейской ассоциации урологов. Результаты. Согласно современным представлениям о патогенезе МКБ, уролитиаз является полиэтиологическим и полипатогномоничным заболеванием. Почечный канальцевый ацидоз (ПКА) является наиболее изученной тубулопатией, приводящей к МКБ. В настоящем литературном обзоре представлены генетические мутации, описаны заболевания и состояния, приводящие к ПКА и МКБ. Подробно приведены особенности течения уролитиаза у пациентов с ПКА. Особое внимание в обзоре отведено методам диагностики ПКА, особенностям лекарственной терапии и метафилактики пациентов с МКБ и ПКА и критериям оценки эффективности проводимой терапии. Заключение. Практикующему урологу важно помнить, что ПКА дистального типа может быть одной из причин литогенеза у пациентов с рецидивирующим течением МКБ. Основными диагностическими критериями являются специфические изменения показателей биохимического анализа крови, суточной мочи, а также особые показатели pH мочи.
