В обзорной статье освещается проблема формирования превентивной кишечной стомы при резекциях прямой кишки с первичным анастомозом. Заболеваемость раком прямой кишки имеет тенденцию к росту, а основным методом лечения остается хирургическая резекция. Несостоятельность первичного колоректального анастомоза значительно увеличивает послеоперационную летальность. Наиболее эффективным способом профилактики грозных осложнений является формиро- вание превентивной кишечной стомы, однако показания, выбор конкретной методики и сроки удаления стомы четко не определены. В статье приводятся сведения о состоянии проблемы, при этом особое внимание уделяется выбору уровня формирования стомы (илеостома или трансверзостома).
