Prognostic factors of efficacy in repeat radiation therapy in patients with continued growth of primary glial brain tumors

Purpose of the study: to evaluate the results of treatment taking into account the presence of certain prognostic factors when conducting a repeat course of radiation therapy in patients with progression of primary glial tumors.Materials and methods. As a result of retrospective analysis, 169 patients with glial brain tumor progression (confirmed by histological examination, MRI-perfusion or PET-CT with methionine) who were hospitalized in 2008-2023 at the Chelyabinsk Regional Center for Oncology and Nuclear Medicine were selected. All patients underwent repeated course of radiation or chemoradiation therapy separately or in addition to surgical treatment. 69 patients (41%) were operated on for progression of the primary tumor. Males predominated in the study (82 males and 76 females). The mean age of patients of both sexes was 45.4±11.7 years. Highly malignant gliomas ( n =129) prevailed in the structure of tumor morphological types. Progression of primary low-grade gliomas was noted in 40 patients. The options of choice for radiation (chemoradiation) treatment were: stereotactic radiation therapy (STRT) on CyberKnife device ( n =48), neutron radiation therapy ( n =38), and 83 patients underwent a course of remote photon radiation therapy.Results. Analyzing the results of repeated radiation treatment for all patients using the Combs scale, we obtained the following data: patients with progression of brain gliomas who underwent a repeated course of radiation therapy, included in group A (from 0 to 1 point) had higher scores of overall survival (OS) compared to patients in group D (from 6 to 7 points) 25 months. and 15 months respectively (p function show_eabstract() { $('#eabstract1').hide(); $('#eabstract2').show(); $('#eabstract_expand').hide(); }
