The aim of this study was to evaluate the effectiveness of adjuvant conformal remote radiotherapy using sequential boost in combination with oncotermia in patients with high-grade gliomas. The study included 158 patients with primary gliomas of a high degree of malignancy (WHO Grade III- IV), who underwent adjuvant chemoradiotherapy in the RNCRR from 2008 to 2021. The average age of patients was 51.8 years. According to the histological examination, glioblastoma (Grade IV)was detected in 118 patients (75%) and Grade III tumors, respectively, in 40 patients (25%). Radiotherapy with sequential boost allows to increase the median proression free survival to 18.07 months. (95% CI from 13,043 to 23,7.), and the adding of oncothermia in the adjuvant course of chemoradiotherapy - up to 22,4 months (CI 95% from 8,641 to 25,593months), compared with 11.8 months (CI 95% from 10,086 to 15,803 months) in the group with radiotherapy in the traditional mode, the differences are statistically significant (p = 0.0003 Log Rank (Mantel-Cox)). The inclusion of oncothermia in the adjuvant chemoradiotherapy regimen with sequential boost was well tolerated by patients and did not significantly increase the toxicity of treatment (p=0.001).
