Multi-modality treatment of primary local and generalized gastric non-Hodgkin lymphomas. Long-term results

Objectives. The main objective of the study was to estimate the effectiveness of different treatment protocols including following treatment modalities: chemotherapy, radiotherapy, surgery in different combinations. In this article two groups of patients were analyzed: local and generalized subgroup. Materials and methods. 125 patients with morphologicaly proven primary non-Hodgkin lymphoma of the stomach were included. We analyzed overall survival, disease-free survival and local recurrence-free survival. Results. In local lymphomas subgroup the complete remission was achieved in 84,3% of the cases, partial remission 5,6%, stabilization 4,5%, progression during treatment 5,6%. In generalized lymphomas subgroup the complete remission was achieved in 55,6% of the cases, partial remission in 27,8%, stabilization - 5,6%, progression during treatment 11%. In local lymphomas subgroup best results were achieved with triple-modality protocol. Chemotherapy alone was less effective than chemotherapy + radiotherapy, especially in the terms of local recurrence free survival (р=0,069). In generalized lymphomas subgroup, chemotherapy plus radiotherapy combination showed better results comparing to chemotherapy plus surgery: 5-year OS, 10 year OS (79% and 76% versus 75% and 75%). It also allowed to achieve better local control results, 5-year and 10 - year local recurrence-free survival 75% versus 100%.
