Современные инструментальные методы диагностики малигнизации язв и рака желудка на ранних стадиях

В странах Западной Европы до 80-90% пациентов с первично выявленным раком желудка имеют позднюю стадию заболевания. Наиболее распространенными методами распознавания малигнизации повреждений слизистой желудка являются фиброгастроскопия с биопсией и рентгеноскопия желудка. Однако их точность определения раннего рака желудка не превышает 80-85%. В статье представлены данные о диагностической значимости современных методов выявления рака и малигнизации язв желудка на ранних стадиях, таких как увеличительная эндоскопия, конфокальная эндомикроскопия, эндоскопические системы NBI и MBI, эндосонография, позитрон эмиссионная томография, компьютерная томография и др. Описаны основные принципы действия. Анализ иностранной литературы показывает, что в настоящее время отсутствуют диагностические методы, однозначно определяющие характер язвы и позволяющие выявлять рак желудка на ранних стадиях со 100%-ной вероятностью. Только сочетанное применение диагностических методов позволяет своевременно выявлять малигнизацию поражений слизистой желудка и точно стадировать течение процесса до операции, тем самым определяя ее объем, вид и прогноз течения заболевания.
