Современные методы малоинвазивного лечения преждевременной (ранней) эякуляции

Введение. Преждевременная (ранняя) эякуляция является распространенной мужской сексуальной проблемой. Несмотря на значительное количество предлагаемых методов, среди врачей до сих пор нет консенсуса по поводу наиболее эффективных способов консервативного и оперативного лечения данной патологии. Материалы и методы. Были проанализированы результаты публикаций отечественных и зарубежных авторов, посвященных различным подходам к оперативному лечению преждевременного семяизвержения: циркумцизио, инъекциям филеров в головку полового члена, различной техники выполнения селективной дорсальной нейротомии, инъекции ботулотоксина в бульбоспонгиозную мышцу, радиочастотной нейромодуляции дорсального нерва полового члена, селективной криоаблации дорсального нерва полового члена. Результаты. Наиболее дискутабельны оперативные техники для лечения преждевременной эякуляции. Показания к операциям, техническое исполнение все еще вариабельны, а результаты трудно предсказуемы. Перспективными представляются селективная дорсальная нейротомия, радиочастотная аблация, криоаблация. Необходима разработка стандартных подходов к подбору пациентов и методике выполнения операций на основе достаточного количества клинических исследований. Заключение. Несмотря на достаточное количество предлагаемых методов хирургического лечения преждевременной эякуляции, необходима стандартизация подходов к подбору пациентов и выбора техники операции.
