Transformation of lowgrade glioma into highly malignant gliomas of the brain

The main goal of this work was to determine the most significant factors affecting the results of treatment in patients with the transformation of relapses of lowgrade glioma into highly malignant gliomas. This study included 20 patients with a morphologically confirmed diagnosis of the transformation of the recurrence of lowgrade glioma into highly malignant glioma, who received treatment in 2000 - 2019 years in Chelyabinsk Regional Clinical Center for Oncology and Nuclear Medicine. Malignization was registered in the period from 10 to 50 months (an average of 44 months). The average age of patients was 40 ± 9.21 years; the ratio of men and women was 2:1. Patients with a primary morphological conclusion of astrocytoma Grade I (n = 13) prevailed, astrocytoma Grade II was diagnosed in seven cases, a relapse of oligodendroglioma was noted in one patient. Eleven patients were diagnosed with relapse with transformation into glioblastoma,nine patients - anaplastic astrocytoma. The median overall survival (OS) for all patients after treatment was 75 months, 1-year OS was 100%, 3-year - 94.7%, 5-year - 71.7%. The median OS after relapse treatment was 24 months. According to the results of our study, the main factors affecting poor prognosis included transformation into glioblastoma and age over 50 years.
