Установлено, что ишемическое прекондиционирование предупреждает возникновение реперфузионной эндотелиальной дисфункции. Однако вопрос о влиянии прекондиционирования на проявления феномена noreflow остается пока открытым. Рецепторные механизмы кардиопротекторного и вазопротекторного эффектов прекондиционирования различаются. Способность прекондиционирования предупреждать реперфузионную эндотелиальную дисфункцию связана с активацией брадикининовых В2-рецепторов и не зависит от стимуляции аденозиновых рецепторов. Вазопротекторный эффект прекондиционирования опосредован через механизмы, относящиеся к активации протеинкиназы С, NOсинтазы, циклооксигеназы, открытию митохондриальных КАТФ-каналов и усилению антиоксидантной защиты сердца.
