Осложнения первичной и повторной буккальной уретропластики у мужчин

Введение. В последнее время отмечена тенденция к выполнению реконструктивно-пластических вмешательств для лечения как первичных, так и рецидивных стриктур уретры. Буккальная уретропластика на данный момент признана «золотым стандартом» лечения стриктур уретры. Однако, несмотря на большое количество работ по оперативной коррекции стриктур мочеиспускательного канала с использованием слизистой щеки, до настоящего времени окончательно не решены вопросы профилактики и коррекции интра-и послеоперационных осложнений буккальной уретропластики. Цель. По данным мировой литературы изучить осложнения, методы их профилактики и коррекции для повышения эффективности первичных и повторных реконструктивно-пластических операций на уретре с использованием свободного аутотрансплантата слизистой щеки. Материалы и методы. Обзор литературы выполнен с использованием библиографических баз данных: Pubmed, Web of Sience, Science Direct, Scopus, Cyberleninka, eLibrary.ru и других за период с 19902022 гг. по следующим ключевым словам: стриктура уретры (urethral stricture), буккальный графт (buccal graft), осложнения буккальной уретропластики (complications of buccal urethroplasty), буккальная уретропластика (buccal urethroplasty), диагностика осложнений буккальной уретропластики (diagnosis of complications of buccal urethroplasty), лечение осложнений буккальной уретропластики (treatment of complications of buccal urethroplasty), анастомотическаяуретропластика (anastomotic urethroplasty), заместительная уретропластика (urethroplasty replacement). Всего были идентифицированы и проанализированы 150 оригинальных публикаций, из них были для литературного обзора отобраны 104 статьи. Результаты. Проведен анализ осложнений первичной и повторной буккальной уретропластики, изучены возможные варианты диагностики и коррекции осложнений реконструктивно-пластических операций на уретре у мужчин с использованием слизистой щеки и проанализировано качество жизни пациентов после хирургического лечения. Заключение. Проведенный анализ научной литературы результатов оперативного лечения первичных и рецидивных стриктур уретры с применением буккального лоскута показывает, что в настоящее время нет единой клинической позиции, которая бы позволяла четко классифицировать возникающие осложнения данного метода уретропластики и способствовала выработке мер их профилактики.
