Современный подход к лечению операбельного плоскоклеточого рака пищевода

Рак пищевода является одним из самых агрессивных злокачественных новообразований желудочно-кишечного тракта, характеризуясь высоким потенциалом злокачественности и плохим прогнозом. Хирургическое лечение является стандартом при локализованной форме рака пищевода, однако результаты самостоятельного оперативного вмешательства при местнораспространенном опухолевом процессе остаются неудовлетворительными. Применение неоадъювантной химиотерапии, как и неоадъювантной химиолучевой терапии приводит к снижению стадии опухолевого процесса, улучшению резектабельности опухоли, а также увеличению показателей общей и безрецидивной выживаемости. Методом выбора при неоперабельном раке пищевода, а также в случае отказа или непереносимости хирургического лечения, является самостоятельная химиолучевая терапия. До недавнего времени адъюватная терапия после RG-резекции не проводилась, однако получены данные, что послеоперационное применение ниволумаба, ингибитора точки иммунного контроля, приводит к достоверному улучшению выживаемости без прогрессирования у пациентов с наличием жизнеспособной резидуальной опухоли. В данной статье представлен обзор современной научной литературы, освещающей особенности лечения операбельного плоскоклеточного рака пищевода.
