Статья научная

The aim was to study changes in the functioning of the hypothalamic-pituitary-gonadal (HPG) axis in BALB/c Nude mice of both sexes with multiple primary malignant tumors (MPMTs). The BALB/c Nude mice (n=84) of both sexes were divided into groups as follows: intact males and females (n=14), a reference group of mice of both sexes with standard subcutaneous inoculation of B16/F10 melanoma (n=14); the main group of mice with the reproduction of the MPMT model (n=14); B16/ F10 melanoma and Lewis carcinoma were inoculated into mice sequentially subcutaneously on two sides: one on the left side and the other on the right side. After 15 days of the growth of the malignant tumors, GnRH, LH and FSH contents were determined by ELISA using standard kits in 1% of the homogenates of the hypothalamic and pituitary tissues, and estradiol content (E2) was determined in 10% homogenates of ovaries and testes, as well concentrations of testosterone (T) and progesterone (P4) were identified in blood serum with the standard RIA kits (Immunotech, Czech Republic). Results. In the hypothalamus in females of the reference and main groups, the level of GnRH increased by 2.5-4 times, and in males it decreased by an average of 3.1 times; in response to those changes in animals of both sexes in the reference group and in the main group only in females in the pituitary gland the level of LH increased by 1.3-1.6 times, however a decrease in FSH in all groups was more pronounced in females: by 7.8-13.7 times and less pronounced in males: by 1.4-1.7 times (р˂0, 05). In the gonads of females, the level of E2 increased, but the levels of T and P4 decreased, while P4 increased in males. Blood parameters did not reflect hormonal changes in the organs. Conclusion. Abnormalities in the HHG axis performance, along with primary immunodeficiency, play a decisive role in overcoming antitumor protection for uncharacteristic malignant tumors.

To know the effect of Diabetes type-II on Eye disease and its risk factors in a tertiary eye care hospital in Karachi. This was a cross-sectional study with retrospective data collection. The data was retrieved from January to June 2019 from the Hospital Information Management System (HIMS) software of the Institute. Non-probability purposive sampling method was used to collect data. Inclusion criteria were all the diabetics type-II patients came for routine check-ups at Diabetes Eye Department irrespective of age, gender and duration of diabetes. Statistical analysis was done using STATA version 13.0 software for windows. A total of 6090 cases were analyzed. Mean age of the participants was 51.01±10.8 years. There were 2954 (48.5%) male and 3136 (51.5%) female observed in this study. Table 1 shows the results of the binary logistic regression analysis with coefficients and their standard errors (S.E.), Wald test values, p values, odd ratios, and 95% confidence intervals for odds ratios. When all explanatory variables treat as continuous variables, we found that all variables which include age, height, random blood sugar (RBS) and duration of diabetes was statistically significant at the significance level 0.05 except weight and fasting blood sugar (FBS) for individual regression slope coefficients. This study reveals findings through binary logistics regression analysis. It indicates that there is an association found in the patients of eye disease with age, height, weight, RBS, FBS. This stated that there were some risk factors observed in these variables related to eye disease. Those who have the disease have risk factor to developed the others associated disease

Shaheen Mehwish, Afzal Muhammad, Mukhtar Madiha

Статья научная

The triage system is meant to be implemented in all (Accidental & Emergency Departments) so as to help and determine the relative priority of individual patient needs. Emergency patients are to be given immediate treatment, while those with non-acute symptoms may have to wait longer (Lucke et al., 2021). Assessment process during the triage need to be balanced with the extent of patient flow, as expanding the assessment process during triage may slow down the patient flow and may lead to delay in emergency service provision (Lucke et al., 2021).Through the use of computer-generated reports, archival data records for two months from the ED of the Bahria International Hospital in Lahore, Pakistan to assess the effectiveness of patients triage flow model. The results of this quantitative study showed that the patient flow model had a considerable temporal effect. Including 16 females, 25 were male and 25% average waiting was more than 240 mints. The ability to decrease patient wait times while enhancing patients quality of care and health outcomes is one of the social change implications. A quantitative study would produce data that would assist medical staff by shortening the wait times for patients.

Effectiveness of modified Manchester triage flow model regarding patients’ waiting time

Effectiveness of modified Manchester triage flow model regarding patients’ waiting time

Статья научная

Triage is a French word meaning to sort or to choose. Triage is therefore a process whereby each patient is prioritized amongst the randomly approaching patients in the Accident & Emergency Department/rescue area for emergency care ( Zachariasse et al., 2021). Sorting of patients into priority categories is often performed by an experienced doctor/surgeon or a senior health professional/nurse. The triage nurse/health professional shall quickly assess the patient’s condition, interpret the clinical features and then exercise interventions in the early phase to prevent deterioration and death (Soler-Sanchis, Martínez-Arnau, Sánchez-Frutos, & Pérez-Ros, 2022). The objective of the triage is to defer a patient who can wait, while give priority to those who are in imminent danger, and whose life can be saved by a timely intervention (Costa, Nicolaidis, Gonçalves, Souza, & Blatt, 2020). The Manchester Triage System is used to maintain a consistent method of prioritising and assessing patients, allowing for thorough audits and improved patient safety. Emergency departments (EDs) are seeing more difficulties globally as a result of an increase in patients and an inability to adjust capacity to meet demand. This is against a background of decreasing hospital resources. Consequently, ED crowding has become a great, international phenomenon (Zachariasse et al., 2021). The Manchester Triage System is one of Europe’s most widely used triage systems, considering five levels to prioritise patients in the ED: level 1 (red), immediate; level 2 (orange), very urgent; level 3 (yellow), urgent; level 4 (green), standard; and level 5 (blue), non-urgent (Brutschin, Kogej, Schacher, Berger, & Gräff, 2021) Patient flow model refers to the movement of patients through health care settings involves the medical care, physical resources, and internal systems needed to get patients from the point of admission to the point of discharge while maintainingand patient/provider satisfaction (Tlapa et al., 2020). The major facility for urgent medical difficulties is a hospital emergency department (ED), which is a complex system with erratic demands.

Obsessive-compulsive disorder frequently have insomnia, and those with acute insomnia who also have mental comorbidities without therapy, they are more likely to develop persistent insomnia. We present a case of OCD with acute insomnia that was successfully treated with early non-pharmacological sleep psychiatry intervention. Sleep is necessary for brain function and the maintenance of cognitive and emotional processes. Insomnia and anxiety problems are common, and they’re linked to a lot of damage and disability. In addition to being strongly comorbid with major depressive illness, there is evidence that sleeplessness and anxiety disorders frequently co-occur. The majority of insomnia psychological therapies include sleep hygiene. In terms of clinical practice, these instructions are a solid place to start. Obsessive-compulsive disorder is associated with sleep difficulties. Sleep difficulties are also common in people with obsessive-compulsive disorder, with up to 48% reporting them. Obsessive compulsive disorder research reveals a link between specific sleep habits and clinical factors such the severity of obsessive-compulsive symptoms, treatment resistance, and the age at which the disorder.

Cerebrovascular accident occurs when the blood supply to the brain is disturbed in some way. As a result the brain cells are deprived of oxygen. It is also known as stroke. It is being observed as a rapidly growing problem and an important cause of illness and death in Saudi arabia. The major problem of cerebrovascular accident is paralysis of swallowing muscles leading to swallowing difficulty which is known as dysphagia. It involves the mouth, throat and esophagus. The goal of this study was to see how swallow treatment affected patients with cerebrovascular accidents at Saveetha Medical College and Hospital in Chennai. The study used a quasi-experimental one-group pre- and post-test design. A total of 30 samples were chosen with care. The Mann Assessment of Swallowing Skills (MASA) was used to examine swallowing ability on regular basis. The results reveal that there is a significant difference in swallowing capacity before and after therapy when using descriptive and interferential statistical approaches. The average score of swallowing ability before swallow therapy was 155.0±16.34, while the average score after swallow therapy was 170.87±11.12. Effects of cardiopulmonary bypass on liver function in patients undergoing cardiac surgery

Reddy A.P., Sunitha R., Thangavel P., Ramkumar J., Miller S.G.

Статья научная

Cardiopulmonary bypass (CPB) might have several systemic impacts on organs such as the liver. This prospective research was designed to investigate the occurrence & significance of this shift in more depth.40 patients of cardiopulmonary bypass were chosen at random for the research. After coronary artery bypass grafting, total bilirubin, direct bilirubin, Alkaline Phosphatase (ALP), SGOT, & SGPT were determined preoperatively & post operation after 24, 48 hours. The liver function tests’ (LFT) post-operative value, in relation to above factors, was compared with preoperative value using t test. The results show a statistically significant increase (p ≤ 0.05) in total bilirubin, direct bilirubin, Alkaline Phosphatase, SGOT, & SGPT on post-operative days 1 & 2 in patients who had complex surgeries & valve replacement surgeries while on CPB. Low cardiac output may be related with a transient rise in hepatic enzyme levels following surgery. Throughout the post-operative period, hepatic enzyme levels were higher in patients with longer CPB durations.

Malchevsky Yury E., Ragimov Aligeidar A.

Статья научная

We have performed the study of the effects made by the automated plasmapheresis application on ischemic heart disease clinical manifestations, echocardiographic indices and heart rate variability in patients with exertional angina III-IV functional class. It has been found that the applied plasmapheresis method is capable to reduce the severity of the disease clinical manifestations and improve the echocardiographic characteristics of the heart, in particular, reduce the level of end-diastolic volume, and increase ejection fraction. The clinical effect is already apparent at the early stages of the treatment and remains unchanged even after 6 months thereof. Also revealed has been the normalization of heart rate variability, indicating a decreased activity of the sympathetic nervous system. It is concluded that the plasmapheresis method can be successfully used in practice by a wide range of hospitals, where patients with ischemic heart disease are monitored, including the ambulatory conditions.
