Obsessive-compulsive disorder frequently have insomnia, and those with acute insomnia who also have mental comorbidities without therapy, they are more likely to develop persistent insomnia. We present a case of OCD with acute insomnia that was successfully treated with early non-pharmacological sleep psychiatry intervention. Sleep is necessary for brain function and the maintenance of cognitive and emotional processes. Insomnia and anxiety problems are common, and they’re linked to a lot of damage and disability. In addition to being strongly comorbid with major depressive illness, there is evidence that sleeplessness and anxiety disorders frequently co-occur. The majority of insomnia psychological therapies include sleep hygiene. In terms of clinical practice, these instructions are a solid place to start. Obsessive-compulsive disorder is associated with sleep difficulties. Sleep difficulties are also common in people with obsessive-compulsive disorder, with up to 48% reporting them. Obsessive compulsive disorder research reveals a link between specific sleep habits and clinical factors such the severity of obsessive-compulsive symptoms, treatment resistance, and the age at which the disorder.

Cerebrovascular accident occurs when the blood supply to the brain is disturbed in some way. As a result the brain cells are deprived of oxygen. It is also known as stroke. It is being observed as a rapidly growing problem and an important cause of illness and death in Saudi arabia. The major problem of cerebrovascular accident is paralysis of swallowing muscles leading to swallowing difficulty which is known as dysphagia. It involves the mouth, throat and esophagus. The goal of this study was to see how swallow treatment affected patients with cerebrovascular accidents at Saveetha Medical College and Hospital in Chennai. The study used a quasi-experimental one-group pre- and post-test design. A total of 30 samples were chosen with care. The Mann Assessment of Swallowing Skills (MASA) was used to examine swallowing ability on regular basis. The results reveal that there is a significant difference in swallowing capacity before and after therapy when using descriptive and interferential statistical approaches. The average score of swallowing ability before swallow therapy was 155.0±16.34, while the average score after swallow therapy was 170.87±11.12. Cardiopulmonary bypass (CPB) might have several systemic impacts on organs such as the liver. This prospective research was designed to investigate the occurrence & significance of this shift in more depth.40 patients of cardiopulmonary bypass were chosen at random for the research. After coronary artery bypass grafting, total bilirubin, direct bilirubin, Alkaline Phosphatase (ALP), SGOT, & SGPT were determined preoperatively & post operation after 24, 48 hours. The liver function tests’ (LFT) post-operative value, in relation to above factors, was compared with preoperative value using t test. The results show a statistically significant increase (p ≤ 0.05) in total bilirubin, direct bilirubin, Alkaline Phosphatase, SGOT, & SGPT on post-operative days 1 & 2 in patients who had complex surgeries & valve replacement surgeries while on CPB. Low cardiac output may be related with a transient rise in hepatic enzyme levels following surgery. Throughout the post-operative period, hepatic enzyme levels were higher in patients with longer CPB durations.

Aims. The aim of the study was to evaluate the effect of total omission of dietary salt intake on BP. Material and methods. The effects of total omission on the consumption of dietary salt over the BP, were evaluated for this study in a group of 30 hypertensive patients between 40-60 years old, uncontrolled, with monotherapy with angiotensin converting enzyme inhibitors or angiotensin II receptor antagonists. BP was recorded over three days at the same time, according to the guidelines of the American Heart Association for correct BP measurement in humans. The average of the three registers measurement was taken as baseline for comparison of the data. The only intervention was the total restriction of table salt and sauces with salt. The patients were followed monthly during the first five months recording BP under the same conditions that basal measurement, after this, were indicated to all the patients the consumption of salt 5g/day (provided by the researchers) and were followed for another five months in the same measurement conditions above...

We have performed the study of the effects made by the automated plasmapheresis application on ischemic heart disease clinical manifestations, echocardiographic indices and heart rate variability in patients with exertional angina III-IV functional class. It has been found that the applied plasmapheresis method is capable to reduce the severity of the disease clinical manifestations and improve the echocardiographic characteristics of the heart, in particular, reduce the level of end-diastolic volume, and increase ejection fraction. The clinical effect is already apparent at the early stages of the treatment and remains unchanged even after 6 months thereof. Also revealed has been the normalization of heart rate variability, indicating a decreased activity of the sympathetic nervous system. It is concluded that the plasmapheresis method can be successfully used in practice by a wide range of hospitals, where patients with ischemic heart disease are monitored, including the ambulatory conditions.

Our results of analyzing effects made by electrode characteristics on the accuracy of the ECG signal parameters formation are presented herein. Analyzed is a correlation between the measured characteristics of the electrodes and a probability value that determines accuracy of the ECG signal parameters formation. With the help of scatter plots demonstrated is the strength of the correlation between the assessed variables. Our analysis of the obtained results shows that the harder is the contact agent in an electrode, the more accurate are the formed ECG signal parameters.

Aims: Enhanced external counterpulsation therapy is a non-invasive, non-pharmacological outpatient treatment option for refractory angina pectoris. Our aim is to evaluate its efficacy in Nepalese refractory angina pectoris patients. Materials and methods: It was single centre prospective study conducted from 2010 August to 2013 December. All thirty one (n=31) consecutive patients, referred for and received 35 hours of treatment were included in this study. The distance covered in six minute walk test before and after the treatment was recorded and compared. Patients were followed each with the questionnaires about their anginal symptoms before and after the treatment. Results: In our study 19(61.3%) were male and 12(38.7%) female. The mean age was 65.7±9.3 years. Most patients had multi vessel disease. Twelve patients had previous history of revascularization. In 6 minute walk test there was significant difference in mean distance covered before and after the treatment. Most patients experienced decrease in the angina symptom. They had decreased in severity and frequency of angina, resulting in decreased use of sublingual nitrates. Conclusion: EECP can be safe and effective treatment option for patients with RAP.

This paper presents the results of a study in which cardiac signal of the first lead of a standard digital ECG system were processed. By filtering in the time domain, the cardiogram was clarified of noise that reduced influence of spectral harmonics by 10-15%. To present and classify frequency characteristics throughout the entire cardio signal, the QS section between the P and T peaks was smoothed. Due to the influence of sharp peaks on the results of spectral analysis, a result considerably differ from the sum of sinusoidal components is observed. The gap between the peaks is interpolated by the Lagrange polynomial since it did not show an influence on the resultant spectrum. The spectral representation revealed peaks with frequencies oddly even, 3.1 Hz and 6.2 Hz, corresponding to the P and T peaks. Based on the obtained results the frequencies corresponding to the remaining peaks of the cardiogram were also classified. The obtained results represent a spectrum of two regular harmonics what allows for further adequate diagnostics of ECG signals.

This work is a comprehensive review of modern aspects of the diagnosis and treatment of endocrine disorders and their impact on reproductive health. The basic principles of the functioning of the human endocrine system are highlighted, including the role of hormonal balance in maintaining the health and functioning of the reproductive system. Various types of endocrine disorders and their effects on reproductive health are also being investigated. In particular, conditions such as hypothyroidism, hyperthyroidism, polycystic ovary syndrome (PCOS), hyperprolactinemia and diabetes, their symptoms, diagnosis and treatment are highlighted. Special attention is paid to the relationship between endocrine disorders and infertility, as well as methods of correcting hormonal imbalances to restore reproductive function. In addition, modern methods of diagnosis and treatment of endocrine disorders are discussed, including laboratory and instrumental methods, molecular genetic studies, as well as advanced approaches in pharmacological therapy, surgical interventions and alternative methods such as active surveillance and lifestyle changes.

Derived is a new modification of hydrodynamic equations of viscous incompressible fluid flowing along the tube with radius changing in time. Obtained are exact non-stationary solutions of these equations generalizing a well-known classic stationary solution for Hagen-Poiseuille flow in the tube with radius constant in time. It is demonstrated that the law of changing the tube radius in time may be determined basing on the condition of minimality of the work expended for flowing the set fluid volume along such a tube during the period of radius change cycle. Obtained is the solution of the corresponding variational (isoperimetric) problem on conditional extremum determining the limits to dimensionless quantity of the cycle duration set by the specified dimensionless value of the flowed fluid volume. Identified is the generalization of well-known model of optimal branching pipeline in which the Poiseuille law modification is used for a new exact non-stationary solution of hydrodynamic equation instead of the law itself. It is demonstrated that the energetically favorable non-stationary modes with negative hydraulic resistance are permissible in certain conditions. The obtained conclusions may be used for development of the hydrodynamic basis of modelling the energy-optimal blood flow realized in the cardiovascular system in norm.

In chemotherapy with various drugs, the tumor cells have an ATP deficiency that contributes to a decrease in their proliferative activity. The aim is to evaluate the level, structure and dynamics of cardiovascular morbidity in the Kabardino-Balkarian Republic for the period from 2014 to 2019. The article describes the structure and dynamics of the general and primary morbidity of the adult population according to the main classes of diseases, the level, structure and dynamics of the general and primary morbidity of the circulatory system diseases from 2014 to 2019. The high prevalence of cerebrovascular disease and arterial hypertension, increase in new cases of acute myocardial infarction, postinfarction cardiosclerosis, cerebral infarction, intracerebral and other intracranial hemorrhage, the effects of cerebrovascular disease on the background of reducing the chronic forms of ischemic heart disease and arterial hypertension primary morbidity. The critical analysis of official statistics as a basis for management decisions is given. Scientific novelty of the work lies in the substantiation of reasonability of clinical and epidemiological research for the planning of preventive measures in the health-care system. As a result, reasonability of the clinical and epidemiological research and the need to implement additional preventive measures is substantiated.

Introduction. The aim of this study is to evaluate the effect of damping coefficient ξ provided by narrow-band notch filters on the quality indicators of the ECG signal processing accompanied by development of a special software package for experimental testing of the cascade filtering system of natural electrical noise and its harmonics. Materials and Methods. For the formation of additive components of signal and noise, test ECG signals from the Massachusetts Institute of Technology were selected as a model, and electrical noise was synthesized taking into account their harmonic nature. The transfer function of the notch filtering system was tuned and calculated. Quantitative quality metrics was used to determine the processing efficiency at the input and output of the signal filtering system. To assess the effect of damping coefficients made on the ECG signal processing system, the method of the linear regression analysis was applied. Methods of structural and procedural programming were used to create a software package for processing a full-scale noisy signal recording. Results. An increase in the efficiency of the ECG signal processing system was revealed when calculating quantitative indicators characterizing the quality solution to the signal filtering problem. It has been found that at values of the damping coefficient ξ = 0.1, the efficiency of the analysis of ECG signal processing in terms of accuracy and noise resistance increases, and the filtering error decreases due to the improvement of the selectivity of the notch filter at the resonant frequency. The effect of the damping coefficient on the quality indicators of ECG signal processing was established, and regression equations were obtained to characterize the adequacy of the designed model for the confidence interval (P = 0.99) at p <0.01. The performance of the developed software package has been demonstrated, which combines a cascade filtering system with a damping coefficient ξ = 0.1, to eliminate the natural electrical interference of the ECG signal and its harmonics. Conclusion. The evidence data obtained in our work, both calculated (theoretical) and practical (experimental) data, confirm the efficiency of the proposed processing approach to the selection of informative components ECG signal under the influence of high-frequency electrical interference and its harmonics. The sequential incorporation of two narrow-band notch filters with a damping coefficient ξ = 0.1 to suppress interference at a frequency of 50 Hz and its harmonics at a frequency of 100 Hz opens up new possibilities for processing ECG signals.
