Статья

Introduction. Low survival and poor quality of life in patients with high-grade brain gliomas (HGG) dictate the need to develop and promote new approaches, including accompanying transcranial magnetic therapy (TMT) with pulse-type magnetic field (PT MF) and ultra-low-frequency magnetic fields (ULF MF), characterized by their non-invasiveness and safety in applied procedures. The key point of these original wave technologies is the construction of algorithms of the actions based on the principles of activation therapy, which was developed as a result of the discovery “Pattern of development of qualitatively differing general nonspecific adaptational reactions of the organism” (Scientific Discovery Registration Certificate No. 158 issued by the Committee on Inventions and Discoveries at the Council of Ministers of the USSR, 1975).

Background. Acute Myocardial Infarction (AMI) is a fatal condition with a subsequent variety of complications. Recently, COVID-19 vaccine has become an essential precaution to avoid infection. However, it is uncommon for AMI to manifest as a result of the COVID-19 vaccination. Methods. Thirty-two years old man, previously healthy, come to the emergency department with a four-hour history of chest discomfort after Covishield (AstraZeneca) vaccination. He was neither hypertensive, diabetic, or smoker and lacked any other typical risk factors for cardiovascular disease. Results. This case describes an adverse response to the COVID-19 vaccination that has the potential to be life-threatening. He had received his first dose of the Covishield vaccination five days prior. Extensive anterior STEMI was diagnosed post-cardiac arrest. He had primary percutaneous coronary intervention (PCI) to recanalize an occluded proximal LAD. Conclusions. Additional screenings for young individuals should be considered before administering the COVID-19 (AstraZeneca) vaccine as a preventive step if reports of significant adverse events in older adults continue.

Unilateral oophorectomy (UOE) of female rats caused severe stress with an increase in the duration of the non-reproductive period (NRP) - the metestrus and diestrus phases, and a shortening of the reproductive period (RP) - the proestrus and estrus phases in the estrus cycle (a symptom of hormonal deficiency). 5 weeks after UOE, the signs of stress persisted: a decrease in the ratio of lymphocytes to the neutrophilic leukocytes in the peripheral blood and the weight ratio of thymus to the adrenal glands, as well as a decrease in the activity of succinate dehydrogenase, which is determined cytobiochemically in peripheral blood cells. A two-week administration of a succinate-containing composition with B vitamins (SCCV) to control and UOE rats increased the ratio of lymphocytes to neutrophils and weight ratio of the thymus to adrenal glands above the intact level - a sign of the “activation” state according to Garkavi and Kvakina and restored the ratio of NRP/RP. At the same time, a significant increase in SDH activity was found in animal peripheral blood lymphocytes. The study demonstrates the possibility to reverse severe stress state to the “activation” state with the help of metabolic therapy containing the combination of SCC and B-Vit.

At the current stage of medical science development there is significant knowledge about various disorders in the tumor process (expression of oncogenes and cytokines, factors of apoptosis, drug resistance, angiogenesis, deficiency of immunoreactivity, etc.). Many of the identified features of the tumor cell are trying to be used as targets for anticancer therapy. However, it is problematic to consider these numerous “details” of the characteristics of a tumor cell as targets for anticancer therapy. This is due to the fact that they do not have a key or independent significance in the appearance of a tumor cell, although they can work in experimental animal models under certain conditions and with prolonged therapy, which at times has little relation to humans

Advanced capabilities of analysis of r/s wave amplitude alternation behavior on ECG as non-linear dynamic system

Chepenko Vladimir V.

Отчет

Heart rhythm abnormalities are of critical importance for an assessment of risks of sudden cardiac death (SCD). “Disorders of heart rhythm are considered as a dynamic process of uninterrupted firing, existence and decaying of elementary sources, initiators of heart arrhythmias.”(Leo Bockeria) If it is a dynamic process, for its description we can use methods of non-linear dynamics that is aimed at time-related SCD risk prediction. This paper considers some issues connected with an application of a specific software module designed for an analysis of R/S amplitude alternation conditions that makes possible to restore the one-dimensional phase portrait of the dynamic system. Our idea is also to offer herein a method how to identify the optimum delay. We have developed methods for calculation of correlation and fractal dimensionality that makes possible to provide a sudden cardiac arrest prediction model.

Sytdykova K.A., Kazantseva Y.V., Sinanyan A.M., Dulkina M.N., Fatenkov G.O., Fatenkova M.A.

Статья научная

This article analyzes age-related risk factors that affect the population health. Risk factors that contribute to poor health are those that increase the likelihood of getting sick or injured. Risk factors are often presented individually, but it is confirmed that they are often interrelated in practice. For example, physical inactivity will lead to weight gain, high blood pressure, and high cholesterol over time. Together, they significantly increase the chance of developing chronic heart and other diseases, and the older a person is, the more various factors impact his health. The study aims to identify age-related risk factors affecting the morbidity of the population. The research was carried out using theoretical methods: the study of scientific articles, literature on the topic, and practical methods: observation and generalization. As a result, it was revealed what risk factors affect the population’s morbidity, the form and degree of their influence on humans, and methods for reducing the influence of these factors were proposed.

Aging and active long healthy life in the context of chronobiology

Zaguskin Sergey L.

Отчет

The challenge of aging and supporting active long healthy life of a human is treated herein in the context of the chronobiological theory of biosystem stability. An increase in the energetic efficiency of the structural regulation in the feedback and adapting process of a biosystem results in a reduction of its homeostatic capacity and inhibition of its biorhythms. The occurrence of pathological desynchronoses provokes further escalation of elimination of some elements in the biosystem and finally aging of the latter. The paper offers description of internal and external causes of desynchronoses and some methods of their removal by normalization of hierarchy of biorhythm periods.

Отчет

Finding an algorithm for determining blood pressure (BP) along the pulse wave contour (PW) is the most difficult task after solving the problem of obtaining the most undistorted PW signal. Attempts to develop such an algorithm inevitably rest on the fact that the measurement of PV in the shoulder area using cuff tonometers (with the method by Korotkoff N. S. or Savitsky N. N.) differs in its physical nature from the cuff-free method of determining blood pressure using an arterial tonometer. However, the requirements for medical accuracy assume that the blood BP deviation tolerance shall be at least equivalent in comparison with the Korotkoff tonometer in accuracy tests regulated by the applicable international standard. In this regard, the computational procedures for processing the PW contour are continuously improved that is the subject of this paper.

Prostate cancer (PC) is the most commonly diagnosed malignancy in men. The standard treatment for localized PC is radical prostatectomy, but recurrence is a significant problem in this case. The main prognostic markers of the disease recurrence are the Gleason score and the preoperative level of the prostate-specific antigen (PSA), which determines the need to identify some key molecules of the PC progression. This can be useful in identifying new prognostic markers or targeted treatments for this pathology.

Aims To overcome numerous contradictions in the knowledge of the blood transportation, a hypothesis was supposed asserting the swirling pattern of blood flow in the heart and main vessels which resembles tornado-like flows and which is widespread in nature. These flows can be exhaustively described by the exact solutions of the nonstationary hydrodynamic Navier-Stokes equations and the continuity equations for the class of viscous swirling flows offered by Kiknadze - Krasnov. Materials and methods Using morphometric studies of human and canine left ventricular casts and Multislice Computed Tomography it has been shown that the trabeculae in the ventricular cavity are oriented along the streamlines of the tornado-like flow of appropriate dimension. Moreover, this principle covers both the diastolic trabeculae of the inlet part of the left ventricle and the systolic trabeculae of the outlet part of the left ventricle. Results It is concluded that the main trabeculae function consists in the structural organization of the intraventricular tornado-like swirling flow of Kiknadze – Krasnov type. Conclusion The hydrodynamic analysis of the blood flow laws is indispensable for the development of new diagnostics criteria in cardiology, cardiac surgery optimization, the design of new implantable and paracorporal devices contacting with blood flow and mathematical and physical modeling of circulation.

Aim. Our aim hereof is to assess the status of infectious morbidity in the pediatric population in the territory of the Russian Federation and the City of Krasnodar in the period 2021-2022 by an example of the State Health Institution “Pediatrics Polyclinic No 2”, Department of Health Care of the City of Krasnodar. Material and Methods. Our work has been completed with use of official statistical data of the Federal Service of the state statistics of the Russian Federation, the database of Rospotrebnadzor of the Krasnodar territory, statistical data set collections of FGBI “The Central Research Institute of the Management and Information of Public Health Care” at the Ministry of Health Care of the Russian Federation. A retrospective analysis was performed using data from the State Report “The state of sanitary and epidemiological welfare of the population in the Russian Federation in 2021”. Statistical processing was carried out using GraphPad Prism 5.0 and Word Excel. Results. According to the data on the sanitary and epidemiological well-being of the population, an increase in the number of cases of infectious and parasitic diseases was recorded in the Russian Federation in 2021. The sharp rise in the incidence of infectious diseases is directly related to the spread of the new COVID-19 infection. The incidence of ARVI among children in 2021 was 68,062.49 per 100,000 that is 15.4% higher than in 2020. In Krasnodar, there were 236,332 patients diagnosed with infectious diseases in 2021. According to the data submitted by Branch No. 2 of the Krasnodar City Pediatrics Hospital at the City Health Care Department, the overall morbidity of children in the first year of life in 2022 has decreased. No cases of diphtheria, pertussis, poliomyelitis, measles, and viral hepatitis B were recorded by the outpatient clinical institution during the study period. As a part of preventive measures, children are routinely vaccinated by the Pediatrics Hospital No. 2; at the end of 2022, 98.3% of children in risk groups had been successfully treated and were allowed to be vaccinated. Conclusion. Over the period 2021-2022, there has been a decline in the incidence of diseases in children in the first year of life in the Krasnodar Region. At the same time, the recorded overall occurrence rate of acute upper respiratory tract infections of multiple and unspecified localization remains high that is due to the difficulty of diagnosing them at the early stages of infection, and, as a result, the effectiveness of their treatment is low that may be accompanied by high mortality.

Статья научная

Autotransplantation of a free flap of the gracilis muscle is currently actively used in reconstructive surgery for persistent prosoplegia, injuries of the brachial plexus, for plastic closure of defects in the upper lip, and in the treatment of pelvic sepsis. This study provides important information about the anatomical features of the gracilis muscle and its neurovascular bundle, which are highly variable. The study of the anatomical and topographic features of this fine thigh muscle was carried out on a sectional study of 25 corpses of both sexes, 50 lower limbs. The present study demonstrates that the median value of the total length of m. Gracilis, the length of its muscular part, as well as the length of the tendon were 452.25 (439.7; 462.0); 225.3(208.1;239.0); 230.5 (213.0; 244.4) mm, respectively. The number of vascular pedicles included in m. Gracilis, ranged from 1 to 5. In 86% of the cases, the deep femoral artery formed the dominant vascular pedicle, and in 14% of cases, it was the medial circumflex artery. Small secondary vascular pedicles originated from the descending genicular artery or the anterior branch of the obturator arter: from the basin of the internal iliac artery. The length of the main feeding artery varied from 76 to 134 mm, the median value was 100.5(90;110) mm, and its diameter ranged from 1.4 to 2.1 mm (M = 1.9(1.8;2, 0) mm. The innervation of the fine muscle of the thigh was carried out by the anterior branch of the obturator nerve, which in 82% of the cases was represented by a single trunk; less often a loose type of structure was observed. The proper nerve was located at a distance of 108.5 (96; 117) mm from the origin of the muscle; its diameter was 2.1 (1.9; 2.2) mm. Knowledge of the topographic and anatomical relationships of the neurovascular bundle with the surrounding formations, variants of its individual anatomical variability are the key to successful reconstructive surgery.
